100 Million Healthier Lives Initiative National Well-Being Survey [communitycommons.org]

We believe well-being is a broader concept than health. Our research experts have identified four components of well-being: physical and mental health as well as social and spiritual well-being. We are dedicated to improving not just health but well-being as a whole.

With that in mind, we are developing a metric called Well-being Adjusted Life Years (WALY), which measures well-being. The data we gather using this survey will allow us to build this metric, which can then provide more information on how to improve health and well-being among different populations.

The survey will take approximately 15 minutes. We will be asking you a variety of questions about your current and future overall well-being, your current and future financial well-being, your health status, and your quality of life. We will also be asking demographic questions such as your age and gender. We recommend you take this survey in a quiet place where you can reflect on your responses.
The responses you provide are completely confidential. Participating in this survey is fully voluntary. You can choose to stop completing it at any point.
As you go through this survey, please time yourself and make note of how long it took you to complete it. You can stop the timer right after you answer the question that asks you what zip code you live in, which is right before you begin the last two questions that ask you for your feedback on the survey. In addition, please make note of which questions, if any, you have trouble with. We will ask you at the end of the survey to list those questions, so we can improve the it for the future.
