I was amazed to learn a bit of how Vermont is responding to this matter, in response to the 2013 Legislature's House Bill 762. I did find the instruction manual that the World Health Organization (WHO) uses with their ACE International Questionaire (and the translation function, as well), to be quite comprehensive. The WHO ACE Questionaire and 'instruction manual/comments' can be found on the WHO website.

Philadelphia AIDS clinic surveys youth drop-ins for ACEs

When the Youth Health Empowerment Project, a Philadelphia program for youth aged 13 to 24, did ACE surveys of those who visited their drop-in center over three weeks, it found that 81 percent had a score of 4 or more.
