Hi Lee Ann, Thanks for that clue! Doing a quick search, there appears to be a variety of efforts that align with the demographic you are seeking to target. I wonder if you might want to reach out to a few folks and see if they have any ideas, suggestions, or leads that may take you closer to your goal?  Your work is really "cutting edge" and you may need to connect the (ACEs) dots for folks who are not yet able to envision what you are trying to achieve. Calhoun County Older Adults data
The Dementia Friendly Communities Initiative is a local effort to inspire the creation of a community that is aware of the impact of dementia and the needs of those affected; where systems of care for persons with dementia well-coordinated and adequate; and where all aspects of the community seek preserve the dignity to ensure a high quality of life for persons with dementia and their families.

The DFC Coalition focuses on awareness, coordination and environment as pillars of impact to address this current community and public health challenge collectively. The Coalition is led by a leadership committee and meets bi-monthly. Contact Karla Fales at 269-441-0976 for more information

Saginaw Valley State University | $352,574 The GRACE Project: Gaining Recovery in Addiction for Community Elders The GRACE Project will expand access to evidence-based treatment for substance use disorders for older adults, particularly through the use of medication-assisted treatment and recovery support services. Teaching strategies like virtual simulations and clinical immersion experiences will help nurse practitioner, social work, occupational therapy, and pharmacy students provide high-quality and compassionate care.

Healthy Aging is one of our proactive grantmaking areas, which means it is open to all eligible organizations. Our Healthy Aging program focuses on comprehensive, integrated, person-centered care for older Michigan residents. The projects funded this round are:
Building a MediCaring Community in the Huron Valley PACE Program | Altarum Institute | $492,574
This project aims to develop and test new strategies for providing comprehensive services to frail older adults, building on the clinical expertise of an established PACE (Program for All-Inclusive Care for the Elderly) program.
