ACEs knowledge survey

Hi I am a graduate psychology student. I am considering the possibility of conducting a survey (of teachers, child care workers, health care providers, etc.) about knowledge of ACEs or if children are screened for these experiences. Does anyone know of an existing survey that has been used for this in previous research or where I could find more information on this? Thank you!
